ABSTRACT

The contents of general practice discipline construction comprise of orientation, team development, personnel training, scientific research, teaching base establishment and management of the discipline, etc. At present, the weakness in above aspects of general practice discipline construction is still the major challenges in most of the primary healthcare institutions. Our experiences in last 10 years shows that the application of Leicester Assessment Package can effectively promote the general practice discipline development for primary care institutions.

ABSTRACT

Objective To explore the correlations between the levels of glycosylated hemoglobin A1c (HbA1c)and N-Terminal pro-Brain Natriuretic peptide(NT-proBNP)in aged diabetes mellitus(DM)patients with heart failure(HF).Methods 127 patients diagnosed as DM complicated by HF and detected HbA1c and NT-proBNP when admitted into hospital,were divided into three groups according to the levels of HBA1C as follows:group A,4%≤HbA1c≤6.9%;group B,7%≤HbA1≤7.9%;group C,HbA1c≥8%.The levels of NT-proBNP were measured and compared.Results There was no statistical difference between group A and B(P>0.05),where as the levels of NT-proBNP in group C increased significantly than that in group A and B(P<0.01);In group C,HbA1c was linear positively correlated to NT-proBNP(r=0.558,P<0.01).Conclusion Aged DM patients complicated by HF,the level of HbA1c between 4%and 7.9%was acceptable.Meawhile,HhA1c should the independent risk prediction factor for aged DM patients complicated by HF,which could be used as the guide to the clinical therapy to improve the prognosis.

ABSTRACT

OBJECTIVE To investigate the status of traumatic infection among earthquake in order to provide reasonable evidence for anti-infective therapy. METHODS Samples were collected from 14 patients with compound fracture or osteofascial compartment syndrome for bacterial,fungal and anaerobic culturing,then according to pathogens to choose antimicrobial susceptibility test (AST). RESULTS Among 14 victims,10 had germ growth in wound secretions,the infection rate was 71.43%. Among 17 samples the bacteria,fungi and anaerobic were isolated in 12,4 and (samples,respectively),the isolated rates were 70.59%,23.53% and 5.89%. The main pathogens for the first culture were Enterobacter cloacae,coagulase negative Staphylococcus (CNS) and Acinetobacter. The Clostridium tetani and C. perfringens were all negative. We also isolated 5 strains of fungi from them 1 strain of Candida. CONCLUSIONS Most of the victims from disaster area have got bacterial infection. Most of them are combined infection,some of the infections are nosocomial infections. The key points of the treatment are to find pathogens immediately and choose a reasonable antibacterial agents according to the AST.

ABSTRACT

BACKGROUND: Chinese herb digan oral liquid promotes manufacture of hematopoietic cells. During irradiative injury, how are the therapeutic effects of intervention with Chinese herbal compound on expressions of stromal cellular adherent molecule (SCAM) and cyclin gene that are closely related to hematopoietic cells?OBJECTIVE: Mouse model with irradiative injury was established to assay the effects of Chinese herbal compound-digan oral lipid on the expression of medullary cyclinD1mRNA and the expressions of hematopoietic stem cells (HSCs), stromal cellular adherent molecular CD45 (SCAMCD45) and vascular cellular adherent moldecular-1 (VCAM-1).DESIGN: Randomized controlled experiment was designed.SETTING: Department of Integrated Traditional Chinese and Western Medicine, Wuhan General Hospital of Guangzhou Military Area Command of Chinese PLA; Institute of Hematology of Tongji Medical College and Institute of Hepatology of Hubei College of Traditional Chinese Medicine.MATERIALS: The experiment was performed in Institute of Hematology of Tongji Medical College and Institute of Hepatology of Hubei College of Traditional Chinese Medicine from January 2001 to June 2002, in which,48 outbred Kunming mice were employed, aged varied from 8 to 12weeks, mass-weighted varied from 18 to 22 g, of either sex. Drugs: 100% digan oral lipid is composed of shudihuang (radix rehmanniae preparata) and zhigancao (radix glycyrrhizae preparata). At ratio of 2/1, water decoction was prepared and concentrated as 100% oral liquid (containing raw herb 1 g/mL). METHODS: Totally 48 mice were randomized into 4 groups, 12 mice in each one. Normal group: no any management. Oral lipid group: Digan oral lipid was applied for gastric perfusion in model with irradiative injury,0.02 ml/g, twice a day. Model group: After modeling, physiological saline was used for gastric perfusion with the same dosage as the above. Batilol group: After modeling, batilol suspension was used for gastric perfusion (0.1 g/kg). Gastric perfusion was done continuously for 9 days in each group. On the 10th day, hybridization in situ was used to assay cyclinD1 mRNA expression of medullary mononuclear cell (MNC), Immuniohisto chemical SABC method was used to assay medullary VCAM-1 expression and flow cytometry (FCM) was used to assay CD45 content.MAIN OUTCOME MEASURES: CyclinD1 mRNA expression of medullary mononuclear cell (MNC) and expression of medullary stromal cellular VCAM-1 and CD45 antigenRESULTS: After modeling, there were 12 mice in normal group and 10 mice in rest groups. In result analysis, 5 mice were randomized in each group. ① In oral liquid group, the expression of medullary cellular cyclinD1 mRNA and VCAM-1 were higher significantly than model group[oralliquid group: (27.34±4.49), (53.94±8.67)% and model group: (16.06±4.57),(31.92±5.01)%, P< 0.01]. ② Expression of CD45 antigen iroral liquid group was higher significantly than model group [(41.38±5.76), (19.24±4.98)%, P < 0.01]. Both digan oral liquid and batilol improved the expressions of cyclinD1,VCAM-1 and CD45 antigen.CONCLUSION: Chinese herbal compound-digan oral liquid enhances the expression of adherent molecule of hematopoietic stem cells, promotes the expression of stroma] cellular adherent molecule and improves the transcription of medullary cellular cyclinD1, which induces hematopoietic cells to be differentiated more maturely and to be transported peripherally so as to supplement blood.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the influence of N-acetyl-L-cysteine (NAC) on pulmonary injury and oxygen stress caused by smoke inhalation injury.</p><p><b>METHODS</b>Rats inflicted with smoke inhalation injury were employed as the model. WBC in BALF (bronchoalveolar lavage fluid), MPO (myeloperoxidase) activity, hydrogen peroxide (H(2)O(2)) content, GSH (glutathione) content and total antioxidant (TAO) capacity in pulmonary tissue were determined.</p><p><b>RESULTS</b>Postinjury WBC in BALF, MPO activity in pulmonary tissue and H(2)O(2) content decreased obviously after NAC treatment. But the pulmonary tissue contents of GSH and ATO increased evidently after the treatment with NAC.</p><p><b>CONCLUSION</b>NAC treatment could ameliorate pulmonary oxygen stress after smoke inhalation injury. AS a result, the pulmonary antioxidant capacity was improved.</p>

ABSTRACT

Objective To determine serum vascular endothelial growth factor (s-VEGF) and serum endostatin (s-endostatin) levels in patients with colorectal cancer after preoperative regional intraarterial chemotherapy (PRAC). Methods Peripheral venous blood was sampled preoperatively and postoperatively, and was assayed for VEGF and endostatin by ELISA. Results Patients' preoperative level of s-VEGF and s-endostatin elevated significantly, which was in close correlation with Dukes' stage. The level of s-VEGF and s-endostatin before chemotherapy elevated significantly compared with that on day 7 after chemotherapy and on postoperational day 1 and 14 after operation. In patients undergoing palliative resection, only the level of s-endostatin before chemotherapy was significantly higher than that on day 1 after operation. Conclusions PRAC, inhibiting angiogenesis, is of antitumor effect, the decrease of s-endostatin after chemotherapy may suggest the combination of chemotherapy and antiangiogenesis for a better antitumor effect.

ABSTRACT

Objective To explore the effect of culture supernatant of Toxoplasma gondii on CD4+CD25+ T cells in mice in vitro. MethodsCD4+CD25+ T cells were separated from spleens of C57BL/6 mice and incubated in the culture superntant of Toxoplasma gondii. The apoptosis percentage of the CD4+CD25+ T cells were detected by FACScan, and the suppression of the CD4+CD25+ T regulatory cells were examined by 3H-TdR incorporation. ResultsCompared with the CD4+CD25+ T cells incubated with RPMI-1640, (36.90?0.36)% CD4+CD25+ T cells took apoptosis after incubated with the culture supernatant of Toxoplasma gondii for 10 hours,and the Annexin-v positive rate increased by (13.60?2.15)%. Compared with RPMI-1640, the culture supernatant of Toxoplasma gondii incubating the CD4+CD25+ T regulatory cells for 5,10 hours reduced their suppressive potential on the proliferation of the CD4+CD25- T cells significantly. ConclusionsSome composition of the culture supernatant of Toxoplasma gondii might cause apoptosis of CD4+CD25+ T cells and as a result, could reduce their suppression on CD4+CD25- T cells.

ABSTRACT

Objective To observe the changes of CD4+CD25+ regulatory T cells in the spleen of mice infected with T.gondii. Methods Twenty-eight female C57BL/6 mice were randomly divided into four groups. Three groups of mice were inoculated intraperitoneally with 104 tachyzoites in 200 ?l sterile PBS. At 2, 4 and 6 days post-infection, the spleens were removed. The expression level of Foxp3 mRNA in splenic CD4+ T cells was quantitated by real-time PCR. The percentage of CD4+CD25+ regulatory T cells in CD4+ T cells was determined by flow cytometry, and the absolute numbers of splenic CD4+CD25+ regulatory T cells and CD4+ T cells were assessed. The fourth group was injected intraperitoneally with 200 ?l sterile PBS as control. Results The relative mRNA level of Foxp3 in splenic CD4+ T cells at day 4 (1.89?0.23) and day 6 (1.79?0.24) post-infection was significantly higher than control (1.00?0.12)(P
